About this event

This interactive session will provide delegates with an overview of care navigation (active signposting) in general practice, how it works, and the benefits to both patients and the practice. Delegates will learn how care navigation is a patient-centred approach which enables patients to see the right person, in the right place, at the right time.

Objectives:

• Understand the principles of care navigation and recognise the value for

practices

• Learn how care navigation can help practice workload

• Develop an awareness of the potential positive impact care navigation can have

• Understand the core competencies required for effective care navigation
